Patient Appointment Information

   Thank you for selecting Champlain Valley Cardiovascular
   Associates for your cardiology care.

   We try to accommodate all requests for specific cardiologists,
   but there may be times when the physician you ask for is not
   available. If this happens, you may see an associate or, if you
   prefer, reschedule your appointment.

Scheduling Appointments
   We realize that our patients' time is valuable, and we make
   every effort to see patients at their scheduled time. However,
   occasionally, our office falls behind schedule. We ask for your
   understanding; we, also, ask that patients provide 24 hour notice
   when needing to cancel or reschedule their appointment. With
   this advanced notice, the time can be scheduled for another
   patient.

  • What to Expect
       On your first visit, we'll ask you to tell us about your medical
       history. We'll give you a comprehensive physical examination, and
       conduct any diagnostic tests necessary to provide you with the
       best possible care. Each office is equipped with state-of-the-art
       diagnostic equipment to provide you with a complete evaluation of
       your heart.

       At the end of your evaluation, the physician will give you their
       impressions and answer any questions you may have. If test
       results are not immediately available, we'll contact you or your
       primary care provider by phone or mail. We will also work
       closely with your family physician and forward a complete record
       of your visit with our recommendations.

       If your condition requires you to be hospitalized for further testing
       or treatment, CVCA's cardiologists will continue to supervise your
       care. More than one of our physicians may see you during your
       hospital stay, but the individualized approach to your care will be
       similar regardless of which physician you see.

    Office Hours/Appointments/Emergencies
       Our regular office hours are 8:30 a.m. to 4:30 p.m., Monday through
       Friday. Patients are seen by appointment only. If you have to
       cancel an appointment, please give us 24 hours notice. We value
       your time, so we make every effort to stay on schedule. However,
       your cardiologist may be delayed due to an emergency or special
       procedure. If this happens, and you are unable to wait, our staff will
       be happy to make another appointment for you.

       If you are experiencing a problem that you feel cannot wait until
       your next appointment (chest pain, for example), you may be
       directed to a hospital emergency room. CVCA cardiologists are
       always available for cardiac emergencies.

       In an emergency, if you need to contact the physician before or
       after office hours, or on the weekends, call the office where you are
       normally seen. Give your name and number, and the nature of your
       problem or concern, to the answering service. Your request will then
       be relayed to our physician on call.

    Insurance and Payment Information
       We are a provider for Medicare and most major insurance plans.
       We provide insurance billing through Sound Management & Billing
       Service, Inc., and we encourage all patients to confirm their
       coverage prior to making an appointment. Co-payments are due
       at the time of the appointment. If the insurance company does
       not pay the full balance, or denies the claim, then any unpaid
       balance is due within 30 days following the appointment.

       Please call us with any questions regarding which insurance plans
       we accept or other billing concerns. Insurance coverage and benefit
       questions should be directed to your insurance company.
